The insurance and billing staff of John Rubin, D.D.S. / Montgomery Plaza Dental, Fort Worth, Texas is here to help with understanding and filing of your dental insurance.

We want to be able to help you maximize your dental insurance. Please help us determine your benefits by bringing your insurance handbook with you to your first appointment. Our office is conveniently located in Montgomery Plaza, 2600 West 7th Street, Suite 184, Fort Worth, Texas.  Phone: (817) 332-5192.  Once we have the above information we make every effort to closely estimate for you what your insurance coverage will be for your treatment. We electronically submit insurance claims on a daily basis and most claims are paid within 30 days.

We will file your insurance and ask that the benefits be paid directly to our office. We will be glad to accept assignment of your insurance benefits. You are expected to pay the amount of treatment that we have estimated insurance will not cover. If the insurance payment varies from the estimate given to you, we expect you to cover the remaining balance due. In the case of overpayment by an insurance company, we will gladly refund the difference or apply the credit to your account.

Facts You Should Know:

Dental insurance is rapidly playing a larger role in helping people obtain dental treatment. Since we strongly feel our patients deserve the best possible dental care we can provide, and in an effort to maintain a high quality of dental care, we would like to share some facts about dental insurance with you.

  • Fact #1: You may receive a letter from your insurance company stating that dental fees are "higher than usual and customary". An insurance company surveys a geographical area, finds the average fee, and then takes 80% of that fee and considers it customary. Included in the fee survey are discount clinics, which bring the average down. Any doctor in private practice will have fees that are considered higher than "usual and customary.
  • Fact #2: Dental Insurance is not meant to be a PAY-ALL, it is only meant to be an aid.  Many plans tell their participants that they will be covered "up to 80% - 100%", but do not clearly specify plan fee schedule allowance, annual maximum or limitations.
  • Fact #3: A dental plan is nothing more than a contract between the employer and the insurance company to partly pay for certain services.  There are deductibles, some services are paid on a percentage while others may not be covered at all.  Your employer buys a contract at a specified premium and includes as many or as few benefits as the employer is willing to pay for.  It is a well known fact within the industry that a higher premium paid by the employer will get you, the patient, a higher "usual and customary" fee schedule.
  • Fact #4: It has been the experience of many dentists that most insurance companies tell their insured that "fees are above the usual and customary fees" rather than saying "our benefits are low".
  • Fact #5: Many routine dental services are NOT covered by insurance carriers.

The professional billing staff of John Rubin D.D.S. / Montgomery Plaza Dental of Fort Worth, Texas  will do all we can to assure you of maximum benefits. We will fill out and file insurance forms at no charge. Please do not be hesitant in asking us any questions about our office policies.  [Phone: (817) 332 - 5192]. We want you to be comfortable in dealing with these matters and we urge you to consult us if you have questions regarding our services an/or fees. If you have questions regarding your insurance, we ask that you contact your company regarding the specifics and details of the plan it is conducting on your behalf.

*Disclaimer: There are times when insurance underpays or denies payment on a claim for a variety of reasons. Any remaining balance not paid by insurance for any reason will become the responsibility of the patient or the patient's guardian if the patient is a minor.
