About Your Visit to John Rubin, D.D.S. / Montgomery Plaza Dental, Fort Worth Texas
"We want you to feel at home in our office, and we will make every effort to make sure that your visits are pleasant experiences. Your health, comfort and convenience are foremost in our minds." -- John Rubin, D.D.S.
YOUR FIRST VISIT: As a new patient, we will review with you the medical and dental history form. Any medications you may be taking or allergies you may have are extremely important to us. This information is held in strictest confidence and is necessary to assure you the best possible dental care.
Generally your first appointment (except in case of an immediate problem) will include a comprehensive examination of the teeth and soft tissues of the mouth. Any necessary radiographs (x-rays) will be taken at this visit in order to help develop a complete diagnosis. After the information has been evaluated, we will then be able to provide you with a proposed treatment plan and fee estimate. Appointments for cleaning and other needed work will then be scheduled for the appropriate amount of time.

RECARE VISITS: We hope that you share in our belief that regular preventative dental health care is a sound investment. If there should be any treatment needed, early care is easier and usually less costly than if treatment is delayed. Your recare examination and preventive cleaning appointments will be scheduled at intervals that are tailored to meet your particular needs. While the primary responsibility for returning for these visits rest with you, we do provide the service of sending you a written confirmation of your continuing care visit in advance. At these appointments, please advise us of any changes in: 1) address, 2) phone number, 3) health, 4) medications, 5) marital status, 6) employment or 7) insurance coverage.
FUTURE VISITS: To give the best and most efficient care we must work with an appointment system. We plan very carefully to review the right amount of time for each patient so that the most amount of dental care can be accomplished in the fewest number of visits. With the patient's best interest in mind, appointment will be scheduled at times best suited for the treatment involved.
Except for emergency situations, you may expect us to be on time, and we would appreciate the same courtesy from you. Your appointment time has been reserved exclusively for you and any change affects many other patients. Should it be necessary for you to reschedule an appointment we require a minimum 48 hours (except in cases of emergency) to avoid a broken appointment fee.
