The mission of John Rubin, D.D.S. / Montgomery Plaza Dental is to provide the best possible dental care for our patients and we make every effort to provide you with a treatment plan tailored to your timetable and budget. In an effort to keep our fees affordable, we do not employ a staff member to work billing and send statements. This allows us to meet a primary goal of the practice - to keep our fees as low as possible.

Our Financial Coordinator will discuss finances with you prior to our office beginning any dental treatment. Payment for treatment is due at the time of your dental visit. We accept Visa, MasterCard, Discover and American Express. Please ask about our cost reduction awards for prepayment of dental fees. We encourage you to discuss with our staff any questions you may have concerning our financial policies.

Montgomery Plaza Dental will gladly work with you to help get your maximum dental insurance benefit available to you. Most dental insurance plans do not cover 100% of your cost of treatment. Because of this, you will be asked to pay your deductible and your co-payment for the charges on the day the service is rendered. Also for all treatment under $100.00, excluding preventive treatment, you will be asked to pay in full. Montgomery Plaza Dental will estimate as closely as possible your coverage, but we can make no guarantee of any estimated coverage. You are expected to pay for the amount of treatment that we have estimated insurance will not cover on the day the service is rendered.

Because the insurance policy is an agreement between you and your insurance company, the ultimate responsibility for all charges lies with you. Any remaining balance not paid by insurance after 60 days, for any reason, will become your responsibility.

• Pre-Payment in full at time your next visit is scheduled, with cash, check or credit card : We offer a 5% courtesy adjustment for payment in full for future treatment when it is scheduled in advance. We're proud to offer you the opportunity to save money on your dental treatment. At no charge, we will gladly process any insurance claim for your direct reimbursement. (Valid for treatment over $500.00)
  
  
• No or low-interest payment plans with no down payment necessary in many cases. For your convenience, Montgomery Plaza Dental is also pleased to offer additional payment methods such as www.carecredit.com by Care Credit (800 365-8295) and www.healthcard.citicards.com from Citibank (866 843-2330), which can be applied for in our office or online!
  At no charge, we will gladly process any insurance claim for your direct reimbursement.
  
  
• Estimated co-payment at visit by cash, check or credit card, along with acceptance of most insurance plans, following benefit verification. Please be aware that Montgomery Plaza Dental cannot guarantee this estimate and that there may be a balance after insurance pays. Whenever choosing this option, we ask that you leave a credit card on file for any balance that may be owed after the claim has been processed.

Orthodontic treatment by John Rubin, D.D.S. is an excellent investment in the overall dental, medical and psychological well being of children and adults. Financial considerations should not be an obstacle to obtaining this important health service. Montgomery Plaza Dental is sensitive to the fact that people have different needs in fulfilling their financial obligations. We provide the same above mentioned payment options with a special orthodontic payment plan provided by www.carecredit.com by Care Credit.

• Pre-Payment in Full:
  A bookkeeping courtesy of 5% is given for direct payment in full at start of treatment by cash, check or credit card resulting in a one-time payment.
  
  
• Office Payment Plan from Montgomery Plaza Dental, Fort Worth, Texas:
  An initial payment is due to the office when treatment begins. A second payment is due upon receiving your aligners.  This fee does not include records, as they are charged separately for your convenience. The balance may be paid through monthly payments withdrawn directly on specified dates from your checking account at your convenience, based upon credit approval.
